What is Social Prescribing?
Many things affect patients’ health and wellbeing. Many patients speak to us about feeling isolated or lonely, being stressed out by work or they have money or housing problems. Sometimes the stress of managing different long-term conditions can get people down.
That’s where social prescribing comes in and it starts with a conversation. It might be the conversation you’ve just had with your doctor, or with another person in the practice team who can then make a referral to a Living Well Co-Ordinator (LWC) who is there to listen to you and put you in touch with the people and activities that might help you to feel better. The LWC will work with you to understand what matters to you to improve your physical and mental wellbeing, for example
- Befriending, counselling and other support groups
- Arts, gardening, creative activities and outdoors
- Healthy lifestyle advice and physical activity
- Meeting people or identifying and attending local groups and activities
- Accessing specialist services and support.
